Pharmacists Aligned in Shared Care Teams

Collaboration among healthcare professionals is essential to providing high-quality patient care, and pharmacists are no exception. Pharmacists Aligned in Shared Care Teams (PACT) and PACT2 are two parts of an educational series that looks to promote the collaboration between pharmacists who are part of a patient’s circle of care working in different settings. PACT is a series of six online learning modules and PACT2 is a series of accredited online modules with live workshops.
PACT2 Content
PACT2 consists of four sessions, each focusing on a different aspect of collaboration:
- Clarifying Roles and Responsibilities: In this session, pharmacists will learn strategies for clarifying their roles and responsibilities, as well as those of their colleagues, to gain a better understanding of the knowledge and skills that other pharmacists have to identify opportunities for collaboration during information gathering, care provision, and follow-up.
- Enhancing Team Functioning: Effective teamwork is crucial to providing high-quality patient care. In this session, pharmacists will learn how to work as a cohesive team, communicate to share knowledge and expertise, and work together to find solutions to complex patient cases.
- Conflict Resolution: Conflicts are inevitable in any workplace, but learning how to resolve them in a positive and productive manner can help to maintain strong working relationships and improve the overall functioning of the healthcare team. In this session, pharmacists will learn effective conflict resolution skills and strategies.
- Collaborative Leadership and Accessing Resources: Improving patient care requires the knowledge and ability to access those services. In this session, pharmacists will learn about the tools and resources that will facilitate collaborative patient care.

By attending PACT and PACT2, pharmacists will be able to:

- Work effectively as part of a team to provide high-quality patient care
- Resolve conflicts in a positive and productive manner
- Provide effective guidance to their colleagues to improve patient outcomes
We are pleased to announce that PACT2 is now open for registration, and workshops will be conducted throughout 2023. PACT1 is not a pre-requisite for participation in PACT2. We encourage pharmacists of all levels of experience and from all practice settings to join us in learning how to collaborate effectively as an intraprofessional team.

About the Pharmacists in Primary Care Network Program
Since 2014, UBC has been developing a model of delivering pharmaceutical care to patients in primary care at its Pharmacists Clinic. This is the model being used in the Pharmacists in PCN Program. UBC has been contracted by the Ministry to lead the first three years of the Program, use their experience to integrate, train and support the 50 PCCPs into team-based primary care and conduct an evaluation of the impact on patient care and the health care system. UBC is also leading a province-wide educational initiative to engage all pharmacists in the shared care of patients.
