Pharmacist Registrant 18 (Feb 27, 2014)
(November 9, 2015 - Partial Limits and Conditions Removed)

The Inquiry Committee, pursuant to Section 36 of the Health Professions Act, has reached an agreement with the pharmacist registrant whereby the registrant consented to undertakings involving regular monitoring for fitness to practice pharmacy. The undertakings include, but are not limited to:

  1. Informing all managers and employers with whom he gains employment of his medical condition and the need to maintain vigilance with respect to recovery and the possibility of relapse, and the limits and conditions on his registration pursuant to the agreement;

  2. Ensuring that all managers and employers with whom he gains employment submits a written statement to the College declaring their awareness of his medical condition and the agreement;

  3. Being restricted from the following roles in his practice:

    1. an owner or manager of a pharmacy;
    2. a director of a corporation that owns a pharmacy; and
    3. a preceptor.

     

  4. Notifying the College of:

    1. all changes in his places of employment as a pharmacist
    2. all changes in residential address and phone number; and
    3. all changes in medical providers

     

The agreement will remain in place for a minimum of two years of continuous pharmacy practice. The name of the Registrant has been withheld in accordance with section 39.3 (4) (a) of the Health Professions Act for the purposes of not identifying the personal health information of the Registrant respecting the condition. The Inquiry Committee is satisfied that the undertakings will protect the public

Lewis, Christopher James (Feb 20, 2014)
(August 23, 2015 - Limits and Conditions Removed)
(AUGUST 23, 2014 - Registration Reinstated)

The Inquiry Committee, pursuant to Section 36 of the Health Professions Act, has reached an agreement with Mr. Christopher James Lewis:

  1. To place limits and/or conditions on his practice effective February 20, 2014. This agreement remains in effect until August 22, 2015. During this time period, Mr. Lewis may not be a manager of The Medicine Shoppe #163 pharmacy.

  2. To suspend his registration as a pharmacist for a six (6) month period commencing February 22, 2014 and ending on August 22, 2014.

The Inquiry Committee considers these actions necessary to protect the public. 

Shahidi , Amir (Feb 7, 2014)

Amir Shahidi has tendered his resignation as a registrant of the College to the Inquiry Committee. The Committee has accepted the resignation and cancelled a Citation pursuant to sections 36(1)(d) and 37(4) of the Health Professions Act since the hearing is no longer necessary nor in the best interests of the College, the registrant or the public.

Pharmacist Registrant 11 (Jan 30, 2014)

Pharmacist registrant has satisfied the terms of an agreement reached with the Inquiry Committee dated December 20, 2011. Limits and conditions on pharmacist registrant’s registration as a pharmacist have now been removed. The pharmacist registrant's name has been withheld pursuant to 39.3(4) of the Health Professions Act

Pharmacist Registrant 20 (Dec 27, 2013)
(September 9, 2014 - Registration reinstated) 

The Inquiry Committee, pursuant to Section 36 of the Health Professions Act, has reached an agreement with the pharmacist registrant to suspend his registration as a pharmacist effective December 27, 2013. The agreement remains in effect until further notice. The Inquiry Committee considers the agreement necessary to protect the public. The pharmacist registrant's name has been withheld pursuant to 39.3(4) of the Health Professions Act.

Pharmacist Registrant 7 (Dec 16, 2013)

Pharmacist registrant has satisfied the terms of an agreement reached with the Inquiry Committee dated November 26, 2010. Limits and conditions on pharmacist registrant’s registration as a pharmacist have now been removed. The pharmacist registrant's name has been withheld pursuant to 39.3(4) of the Health Professions Act.

De Guia, Aldwin Ramirez (Dec 5, 2013)
(OCTOBER 17, 2015 – LIMITS AND CONDITIONS REMOVED)

Aldwin De Guia has completed a term of suspension which ended on December 4, 2013, and his registration has been reinstated as of December 5, 2013.

Ahn, Hye Won (Nov 10, 2013)

Hye Won Ahn has completed a term of suspension which ended on November 9, 2013, and her registration has been reinstated as of November 10, 2013.

(DECEMBER 5, 2013 - REGISTRATION REINSTATED)

The Inquiry Committee, pursuant to Section 36 of the Health Professions Act, has reached an agreement with Mr. Aldwin Ramirez De Guia:

  1. To place limits and/or conditions on his practice effective October 16, 2013. This agreement remains in effect until October 16, 2015. During this time period, Mr. De Guia may not be a manager, director, or owner of a pharmacy or hold shares in any corporation that owns a pharmacy.

  2. To suspend his registration as a pharmacist for a 30 day period commencing November 4, 2013 and ending on December 4, 2013.

The Inquiry Committee considers these actions necessary to protect the public.

Wong, Soon (John) Him (Oct 15, 2013)
(April 16, 2014 - Registration reinstated)

The Inquiry Committee, pursuant to Section 36 of the Health Professions Act, has reached an agreement with Mr. Soon Him John Wong:

  1. To place limits and/or conditions on his practice effective October 15, 2013. This agreement remains in effect until October 15, 2015. During this time period, Mr. Wong may not be a manager, director, or owner of a pharmacy or hold shares in any corporation that owns a pharmacy.

  2. To suspend his registration as a pharmacist for a 180 day period commencing October 17, 2013 and ending on April 15, 2014.

The Inquiry Committee considers these actions necessary to protect the public.
