Pharmacist Registrant (Oct 11, 2012)

On October 11, 2012, a Panel of the Discipline Committee, pursuant to s. 39.1(1) and (2) of the Health Professions Act, has decided to take no further action with regards to the pharmacist registrant. The Discipline Panel further directed the Registrar to withhold all of the information otherwise required to be included in the public notification, pursuant to s. 39.3(4) of the Health Professions Act, as the Discipline Panel was satisfied that the privacy interests of the pharmacist registrant outweigh the public interest in public notification of the information.

Lee, Benton (Aug 18, 2012)

Benton Lee has satisfied the terms of an agreement reached with the Inquiry Committee dated April 6, 2012. Limits and conditions on Mr. Lee’s registration as a pharmacist have now been removed. 

Tejani, Diamondali Alladin (Aug 16, 2012)

Diamondali Alladin Tejani has satisfied the terms of an agreement reached with the Inquiry Committee dated June 29, 2012. Limits and conditions on Mr. Tejani’s registration as a pharmacist have now been removed.

Wong, Daniel (Jul 16, 2012)

The Inquiry Committee, pursuant to Section 35(1)(a) of the Health Professions Act, has imposed limits and conditions on the practice of pharmacy by Mr. Daniel Wong, effective immediately, pending a hearing of the Discipline Committee. The Inquiry Committee considers this action necessary to protect the public. 

Ghag, Sukhvinder (Jul 16, 2012)

The Inquiry Committee, pursuant to Section 35(1)(a) of the Health Professions Act, has imposed limits and conditions on the practice of pharmacy by Mr. Sukhvinder Ghag, effective immediately, pending a hearing of the Discipline Committee. The Inquiry Committee considers this action necessary to protect the public.

Tejani, Diamondali Alladin (Jul 16, 2012)
(August 16, 2012 - Limits and conditions removed)

The Inquiry Committee, pursuant to Section 36 of the Health Professions Act, has reached an agreement with Mr. Diamondali Alladin Tejani to suspend his methadone dispensing privileges for a 30 day period commencing July 16, 2012 and continuing until August 15, 2012. During this time Mr. Tejani may practice pharmacy, however he is prohibited from participating in any activity which in any way relates to the dispensing of methadone to patients. The Inquiry Committee considers the agreement necessary to protect the public.

Ladha, Aziza (Jul 10, 2012)

The Inquiry Committee, pursuant to Section 36 of the Health Professions Act, has reached an agreement with Ms. Aziza Ladha to place limits and/or conditions on her registration as a pharmacist effective July 10, 2012. The agreement remains in effect until July 10, 2014. The Inquiry Committee considers the agreement necessary to protect the public.

Mirza, Hajra (Jun 25, 2012)
(July 9, 2013 - Limits and conditions removed) 

The Inquiry Committee, pursuant to Section 36 of the Health Professions Act, has reached an agreement with Ms. Hajra Mirza to suspend her registration as a pharmacist for a 30 day period commencing June 25, 2012 and ending on July 25, 2012. The Inquiry Committee considers the agreement necessary to protect the public.

Keller, Jason William (Jun 16, 2012)

The Inquiry Committee, pursuant to Section 36 of the Health Professions Act, has reached an agreement with Mr. Jason William Keller to place limits and/or conditions on his registration as a pharmacist effective June 16, 2012. The agreement remains in effect until June 15, 2014. The Inquiry Committee considers the agreement necessary to protect the public.
