Pharmacist Registrant 15 (Jul 9, 2013)

The Inquiry Committee has reinstated pharmacist registrant’s registration which was suspended for an indefinite period on March 22, 2013. Pursuant to Section 36 of the Health Professions Act, the Inquiry Committee has reached an agreement with pharmacist registrant to place limits and conditions on his registration as a full pharmacist, effective July 9, 2013. This agreement remains in effect until July 9, 2016. The Inquiry Committee considers the agreement necessary to protect the public. The pharmacist registrant's name has been withheld pursuant to 39.3(4) of the Health Professions Act

Laal, Ali (Alejandro Farris and Alejandro Fernandez) Asghar (Jun 26, 2013)

(known to use the following aliases: Alejandro Farris, Alejandro Fernandez, Jensen Farris, Alistair Farris, J. Alistair Lail)

On June 26, 2013, a Panel of the Discipline Committee, pursuant to s. 39(1)(c) of the Health Professions Act, found Ali Asghar Laal of Victoria, British Columbia, Registration No. 08811 (the "Registrant") guilty of professional misconduct for:

  • Having inappropriate personal relationships with two patients, one of which included having sexual relations with the patient; and
  • Keeping a loaded handgun hidden within the licensed pharmacy of which he was the owner and pharmacy manager.

The Panel also found, pursuant to s. 39(1)(a) of the Health Professions Act, that the Registrant had not complied with the Bylaws to the Pharmacy Operations and Drug Scheduling Act by maintaining outdated pharmaceutical products in the licensed pharmacy of which he was the owner and pharmacy manager, and keeping the outdated products as part of the active pharmacy inventory; and failing to notify the College that the licensed pharmacy of which he was the owner and pharmacy manager had been renovated.

Al-Ahmad, Hisham Nayef (Jun 25, 2013)

Hisham Al-Ahmad has satisfied the terms of an agreement reached with the Inquiry Committee dated November 30, 2012. Limits and conditions on Mr. Al-Ahmad’s registration as a pharmacist have now been removed.

Pharmacist Registrant 15 (Mar 22, 2013)
(July 9, 2013 – Registration reinstated)

The Inquiry Committee, pursuant to Section 36 of the Health Professions Act, has reached an agreement with pharmacist registrant to suspend his registration as a pharmacist for an indefinite period pending further disposition by the Inquiry Committee. The Inquiry Committee considers the agreement necessary to protect the public. The pharmacist registrant's name has been withheld pursuant to 39.3(4) of the Health Professions Act.

Pharmacist Registrant 10 (Jan 25, 2013)

Pharmacist registrant has voluntarily changed registration status to Former Pharmacist, effective January 25, 2013. As such, the registrant is no longer subject to the practice limits and conditions as described in the agreement reached on April 24, 2011. Should the registrant at a later date request to reinstate registration to Full Pharmacist, the registrant’s request will be subject to review by the Registration Committee. The registrant’s name has been withheld pursuant to s. 39.3(4) of the Health Professions Act.

Dhinsa, Lakhvir Singh (Jan 11, 2013)

On January 11, 2013, a Panel of the Discipline Committee, pursuant to s. 39.1(1) and (2) of the Health Professions Act, found Lakhvir Singh Dhinsa of Surrey, British Columbia, Registration No. 10961 (the "Registrant") guilty of professional misconduct for actions committed in Great Britain, resulting in a cancellation of his license in that jurisdiction. The Panel issued an order, pursuant to section 39(2) of the Health Professions Act, to immediately cancel the Registrant's registration.

Al-Ahmad, Hisham Nayef (Dec 5, 2012)
(June 25, 2013 - Limits and conditions removed)

The Inquiry Committee, pursuant to Section 36 of the Health Professions Act, has reached an agreement with Mr. Hisham Nayef Al-Ahmad to suspend his narcotic dispensing privileges for a 30 day period commencing at 0001 Hours on December 5, 2012 and continuing until 2400 Hours on January 3, 2013. During this time Al-Ahmad may practice pharmacy, however he is prohibited from participating in any activity which in any way relates to the dispensing of narcotics to patients. The Inquiry Committee considers the agreement necessary to protect the public.

Pharmacist Registrant (Oct 11, 2012)

On October 11, 2012, a Panel of the Discipline Committee, pursuant to s. 39.1(1) and (2) of the Health Professions Act, has decided to take no further action with regards to the pharmacist registrant. The Discipline Panel further directed the Registrar to withhold all of the information otherwise required to be included in the public notification, pursuant to s. 39.3(4) of the Health Professions Act, as the Discipline Panel was satisfied that the privacy interests of the pharmacist registrant outweigh the public interest in public notification of the information.

Lee, Benton (Aug 18, 2012)

Benton Lee has satisfied the terms of an agreement reached with the Inquiry Committee dated April 6, 2012. Limits and conditions on Mr. Lee’s registration as a pharmacist have now been removed. 

Tejani, Diamondali Alladin (Aug 16, 2012)

Diamondali Alladin Tejani has satisfied the terms of an agreement reached with the Inquiry Committee dated June 29, 2012. Limits and conditions on Mr. Tejani’s registration as a pharmacist have now been removed.
